To answer your questions:

1) I think we should left-justify everything;

2) Words in small caps should be kept completely in upper case;

3) Words in smaller fonts should be surrounded by small tags:

Jere, thanks for improving the page by making the word "confidential" a smaller size, as it was in the original; and for showing how you accomplished that. It only took me five tries to figure out how to sign this note the same way you signed yours. Wasn't sure whether to use quote marks or not, whether the tilde(s) indicated where words should be written or whether the tildes were actual commands that tell the computer to recognize the person who is signed in and print his name. Anyway, I had the good sense not to save the edit until after all those tries had succeeded.
